Professionals about Inclusion Health – Public Health South Tees

We have been asked to circulate this survey amongst our audience that may be healthcare staff or the voluntary sector.

This survey is for professionals working in South Tees who support people from inclusion health groups.

The findings will inform a Health Needs Assessment, which aims to better understand their health needs and priorities.

The views of healthcare, voluntary sector, and local authority professionals are important for supporting this.

All responses will be anonymous.

This survey will take around 5-10 minutes to complete and will close on Friday 12th June at 5pm.
